A. For new patients, please print and complete the new patient forms. Please bring your insurance card and any prescription medications, even if prescribed by another physician.

Q. How early should I arrive for my appointment?

A. Please arrive 10 minutes early for your appointment to verify and update any changes to your personal insurance information. You also need to allow time for parking due to construction at the hospital. If you are a new patient please arrive 15 minutes early to your appointment to complete any necessary paperwork.

Q. Can I pay on my past due balance at my appointment?

A. Yes, we accept cash, personal checks, Visa, MasterCard, and Discover cards.
