Appointment Information

To make an appointment, please call (704) 403-1766 during normal office hours. New patients need to have their primary care physician call to schedule their first appointment with the clinic.

First Visit
Each new patient is asked to print and complete the new patient forms.

Please bring your insurance card and any prescription medications to your appointment. Please arrive 15 minutes early to complete registration and other paperwork.

Cancellation Policy
Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.
